Payment Options

Residents of Outgamie, Winnebago, and Calumet counties may receive services at the clinic.

Medicare, Medicaid and all other medical insurances are accepted.

Discount services are available for residents without insurance with proof of family income. Items needed for income verification include paystubs equalling one month of work, tax return or W-2, and photo id.

If unemployed, a letter is required from whomever is providing shelter and support.
