Scheduling an Appointment

To schedule an appointment at our facility, please call (920) 731-7445.

Make sure you come to the clinic prepared. We accept Medicaid, Medicare, and other insurance. If applicable, please bring your insurance card.  If you are new to the clinic, a pre-registration visit prior to the medical appointment is encouraged.

Discounts are available to residents without insurance; there is a co-pay expectation for each service.  Please bring proof of family income, address, photo ID, and social security number when visiting the health center. If you are unemployed, a letter is required from whomever is providing shelter and support

Co-pays are collected at the time of service, so please be prepared to pay at our front desk. In addition to cash, we accept checks, money orders, and most credit cards.

No patient needing services will be turned away - the clinic has flexible payment plans based on the ability to pay.
